Complete these forms from the actual tender pack — do not use generic versions. All must be signed and dated.

SBD 1 — Invitation to Bid (signed)

MANDATORY

The cover page — must be signed and dated by authorised signatory

SBD 3.1 / 3.2 / 3.3 — Pricing Schedule

MANDATORY

Complete all pricing items. No blanks, no crossing out. Use the correct version (firm/non-firm/professional)

SBD 4 — Declaration of Interest

MANDATORY

Declare relationships with state officials. Complete even if N/A

SBD 6.1 — Preference Points Claim

MANDATORY

Claim B-BBEE points accurately. Must match your uploaded B-BBEE certificate

SBD 6.2 — Local Content Declaration

SBD 8 — Past SCM Practices

Section 4: Final Submission Checklist

Original + required number of copies (check tender document)

Sealed envelope(s) labelled with bid number, company name, closing date
